The Traveller Health Declaration is a self-declaration form required by the Ministry of Health of the Republic of Angola from every traveller arriving in or transiting through the country. Please complete it before departure.
The information collected is used exclusively for cross-border communicable disease control and for health screening at the point of entry. It is not a visa and does not replace any immigration requirement.
Find an existing submission by Declaration ID and correct any detail before you travel.
Edit DeclarationEach traveller, including infants and minors, requires an individual declaration. For minors, the declaration is completed by a parent or guardian. Keep your Declaration ID: it is the only way to edit your submission afterwards.
